About Novel Coronavirus Vaccination

  • Novel Coronavirus Vaccination tickets (coupons) have been sent to people aged 12 and above.
  • The Novel Coronavirus (COVID-19) vaccine requires two injections.
  • A prior reservation is required for the vaccinations. It is free of charge for both times.
  • At the moment, all those aged 12 and above with the Novel Coronavirus Vaccination tickets (coupons) are able to make the reservation.
  • Reservations are accepted for all with the tickets (coupons) if there is a vacancy.
  • A parent needs to accompany the child, when a child under the age of 16 is getting vaccinated.
    Note: Must be at least 12 years’ old on the day of vaccination.
  • For those who are not 12 years’ old yet, the tickets (coupons) will be posted on their 12th birthday date.
  • Reservation starting dates had been planned to be notified by postcards to all. However, please note that the postcards will not be sent to people aged 49 and under.

Reservation of Novel Coronavirus Vaccination

You can choose from the choices of large venues or hospitals to receive your vaccine.

How to make the Reservation

Reservation can be made either by phone or by website.

The followings are needed when you make a reservation:

  • Vaccination ticket (10-digit ticket number)
  • Name
  • Date of birth
  • Telephone number

Things to keep in mind when making a reservation:

  • Two doses of the vaccine are required. The second dose will be given 19 to 21 days after the first vaccination.Reservations need to be made for both doses.
  • If you wish to cancel a reservation, please do so at least two days in advance.For the change of the schedule, you must cancel all your reservations once and make the reservations anew again.
  • If you plan to be vaccinated at a large-scale vaccination site in Otemachi in Tokyo, please be careful that duplicate reservations of both sites at Tokyo and Urayasu City may not be made.
    In addition, reservation of the two doses must be made for the same location, either at the large-scale Tokyo site or at Urayasu City (if you are making the reservation of the first dose at Tokyo, then please make the reservation of the second dose at Tokyo as well, and the same goes for Urayasu City).

  • Urayasu City is providing vaccinations at the following facilities.
  • Please make a reservation for the location and time of your choice.
  • Vaccinations will be given every Wednesday, Thursday, Saturday, and Sunday.

Novel Coronavirus Vaccination for Pregnant Women

Reservation priority for the vaccination is given to pregnant women and members living in the same address.

Priority is given for the vaccination at large venues.

What to Bring on the Vaccination Day

Pregnant women:

  • Vaccination ticket (coupon)
  • Mother and Child Health Handbook
  • An identity verification document (such as a Residence card, driver’s license, health insurance card)

Members living in the same address:

  • Vaccination ticket (coupon)
  • Mother and Child Health Handbook (copy)
  • An identity verification document (such as a Residence card, driver’s license, health insurance card)

Please Note Before Receiving the Vaccination

The following persons should ask the doctor at their regular clinic whether or not they could receive the vaccination.

If needed, please come with someone who could help you with the Japanese language.

  • Person who is a regular patient at a hospital.
  • Person who is worried about getting vaccinated due to medical concerns.
  • Person who had fever or allergies such as rash within 2 days after injection in the past.
  • Person who has had seizures in the past.
  • Person who might be allergic to the ingredients contained in the vaccine.
