Obtaining residence record documents and personal seal registration certificates

Your residence record confirms your address, name, date of birth and nationality. A personal seal registration certificate confirms that a personal seal is yours.

You must pay a fee to obtain copies of your residence record or personal seal registration certificate.For more information, contact the Civic Affairs Section.

Where to obtain the documents and hours of operation

• Civic Affairs Section, Urayasu City Hall 1F (1-1-1 Nekozane, Urayasu City)
  Monday to Friday and on Sunday, 8:30 a.m.to 5 p.m. (closed on national holidays and year-end/New Year holidays)
• Station City Service Centers
  • Maihama Station City Service Center (Maihama Station North Exit 2F, 25-2 Maihama, Urayasu City)
  • Shin-Urayasu Station City Service Center (Shin-Urayasu IL MARE 2F, 1-2-1 Irifune, Urayasu City)
  • Urayasu Station City Service Center (Seiyu Part 2, 1F, 1-13-25 Kitazakae, Urayasu City)
  Monday to Friday, 8:30 a.m.to 5 p.m.(closed on national holidays, and year-end/New Year holidays)

What to bring

• Personal identification (residence card, driver’s license, etc.)
• If a representative is coming, the person must bring your proxy letter

300yen per document.

If you have a My Number card, you can also obtain copies of your residence record and personal seal registration certificate at convenience stores or by mail.
